ABSTRACT

We report a case of chromoblastomycosis caused by , which was successfully treated by long-pulsed 1064 nm Nd: YAG laser combined with terbinafine. A 60-year-old man was admitted for the presence of a 30 mm×40 mm erythematous plaque on the dorsum of his right hand for about 10 months without any subjective symptoms. Both microscopic examination and tissue biopsy of the lesion showed characteristic sclerotic bodies of chromoblastomycosis. Lesion tissue culture on SDA at 26 ℃ for 2 weeks resulted in a black colony, and slide culture identified the isolate as Fonsecaea species. ITS sequence analysis of the isolate showed a 99% homology with strain KX078407. The susceptibility of the isolate to 9 antifungal agents was determined using the microdilution method according to the guidelines of CLSI M38-A2 protocol, and terbinafine showed the lowest MIC (0.125 μg/ml). We subsequently established a Wistar rat model of chromoblastomycosis using the clinical isolate and treated the rats with long-pulsed 1064 nm Nd: YAG laser (pulse width of 3.0 ms, fluence of 24 J/cm, spot size of 3 mm, frequency of 4 Hz, repeated 3 times at an interval of 30 s) twice a week for a total of 8 sessions. Although the laser treatment alone was not able to eliminate the fungi, histopathological examination showed the aggregation of numerous lymphocytes in the local affected tissue, indicating an immune response that consequently facilitate the regression of the lesion. The patient was successfully treated by long-pulsed 1064 nm Nd: YAG laser once a week combined with terbinafine (0.25 /bid) for 8 weeks, and follow-up for 20 months did not reveal any signs of recurrence.

ABSTRACT

Objective To investigate the correlation between serum sFas and sLOX-1 with occurrence and development of acute coronary syndrome (ACS).Methods A total of 52 patients definitely diagnosed ACS (ACS group) by coronary artery angiography (CAG) were enrolled, including 23 cases of unstable angina (UA group) and 29 cases of acute myocardial infarction (AMI group),and contemporaneous 58 cases of non-coronary arterial stenosis confirmed by CAG were selected as the control group (NC group). The serum levels of sFas and sLOX-1 were measured by enzyme linked immunosorbent assay. Results Compared with the NC group, the serum levels of sFas and sLOX-1 in the ACS group were increased, the serum levels of sFas and sLOX-1 in the AMI group and UA group were higher than those in the NC group, moreover which in the AMI group were higher than those in the UA group (P<0.01).The serum sFas level in the ACS group was positively correlated with the sLOX-1 level (r=0.825,P=0.001),but both had no obvious correlation with the serum levels of CK-MB and cTnⅠ (P>0.05).Conclusion High levels of serum sFas and sLOX-1 may be the risk factors of ACS.

ABSTRACT

We report a case of cutaneous and subcutaneous phaeohyphomycosis caused by Exophiala jeanselmei after renal transplantation in Guangdong. A 66-year-old man who had a renal transplantation 6 years ago was admitted in October 2011 for the presence of 16 nodules (0.5-1.5 cm) found on his right middle finger, wrist and forearm for 5 months. Microscopic examination of the purulent exudate showed segmented and branched brown mycelium, and tissue biopsy and PAS staining showed fungal hyphae. The isolate was processed for morphological identification and molecular sequence analysis. A black colony was found after culture of the isolate on SDA at 26 degrees Celsius;, and small culture identified the isolate as Exophiala jeanselmei. ITS sequence analysis of the isolate showed a 100% homology with Exophiala jeanselmei. E-test strip was used in drug sensitivity test, and the isolate was sensitive to amphotericin B, voriconazole, itraconazole and fluconazole, but resistant to 5-flucytosine and caspofungin. Good response was obtained with surgical intervention, local injection and systemic antifungal treatment.

ABSTRACT

Objective To elucidate the significances of health education in the prevention and treatment of gout and hyperuricemia.Method Health education scientific knowledg was done through approaches of specific proclamation in clinic out patients, screening the directly-related members of patient family, training medical staffs and popular vehicle propaganda etc. Results Health education improved the diet structure and life mode of patients, promoted early diagnosis and treatment of patients. It also had favourness in timely detection of other patients in directly-related members of ones family. Conclusion Health education combined with conventional pharmacological therapy could produce a better efficacy than that of single pharmacological ones.

ABSTRACT

Objective To study the value of intraoperative cholangiography(IOCG) in cholecystectomy for gallbladder stone. Methods The IOCG data of 694 cases in the Affiliated Hospital, Yanan University Medical School were retrospectively analysed. Results Among 694 cases with IOCG, 65 had positve findings. Bile duct stone occurred in 44 patients, residnal stone in cyst duct in 7, bile duct injury in 2, and Mirizzi syndrom in 3. Among the 44 cases with bile duct stone, the age of 4.3% cases was 22~50 years old, 10.3% older than 50 years. The incidence of bile duct stone occurring after 50 years old increased with age. The accurate rate of IOCG was 98% in this series. Conclusions IOCG can not only find residual stone, but also find bile duct abnormality and bile duct injury in time, if correct imagic technique is applied.

ABSTRACT

Objective To summarize the clinical experience of laparoscopic cholecystectomy(LC) for acute cholecystitis. Methods Clinical records of 58 cases of acute cholecystitis treated by LC from March 1998 to May 2004 were respectively reviewed. Results Intraoperative cholangiography was conducted in 6 cases,5 of which were found to have common bile duct stones.Of the 5 cases,2 underwent LC combined with choledoscopic choledochotomy, stone removal and T tube drainage; the other 3 cases had ill defined relationship of Calot′s triangle and underwent conversion to open choledochotomy with stone removal and T tube drainage. In 52 cases were diagnosed as simple gallbladder stones, LC was successfully accomplished in 50 cases and conversion to open surgery was required in 2 cases because of serious inflammatory adhesions. In this study 1 case had jaundice after operation and 3 cases had leakage of bile, and all recovered on conservative treatment . Conclusions LC can be performed safely in the majority of cases of acute cholecystitis.
